Travel

PhD students are allotted $1500 per academic year to cover the cost of degree-related travel. Travel reimbursement forms are available from Malinda Sloan (malinda.sloan@colostate.edu).

Required Graduate School Forms

Students are responsible for the completion of these forms and adhering to the required submission deadlines. If you have questions about the forms, please contact the PhD program director or coordinator. An up-to-date list of required graduate school forms may be found on the Graduate School website.

GS6, Program of Study -Due in third semester
GS16, Report of Qualifying Exam – Due 2 working days after
GS24, Report of Final Examination Results (i.e., the dissertation defense) – Due 2 working days after exam
GS25, Application for Graduation – Due in graduating term
GS25B, Departmental Requirements Clearance – Due in graduating term
GS30, Thesis/Dissertation Submission – Due in graduating term
